As part of your passport application, you will need to provide proof of U.S. Citizenship.
You must provide a certified copy or the original of one of the below:

Any original documents submitted will be returned after the completion of your passport.
If using your passport, it must be your most recently issued 5 or 10 year validity passport book or card. To be used as a proof of citizenship, it must be less than 15 years from the issue date. | |
The birth certificate should be issued by the office of vital statistics of the state, county, or city where your birth occured, show your full name, date, and place of birth, list the full names of both parents, bear an embossed, impressed or raised seal and signature of the issuing authority, and indicate a registration or file date that is within on year of your birth. | |
A hospital-issued birth certificate is not the certified copy that is needed. | |
